HIV Case Management Services

Our case management program is committed to helping people living with or affected by HIV/AIDS. Our case managers provide emotional and practical support to individuals facing many issues due to a diagnosis of HIV or AIDS. Our professional staff is dedicated to the health and wellness needs of people living with HIV or AIDS through support, understanding and hope. OUR case managers can help in the following ways:

• Assist with referrals to community services, specialized medical care and mental health services.
 
• Coordinate transportation services to and from medical appointments.
 
• Assist with securing medications through state programs, pharmaceutical, compassionate care programs and local resources.
 
• Coordinate volunteer services to provide emotional and practical support.
 
• Provide support groups and educational programs for people living with HIV/AIDS.
 
• Provide temporary, emergency financial assistance with housing, utilities, food, clothing and other essential needs.
 
• Coordinate services with home health and community health providers as appropriate.
 
• Assist with financial planning with medical bills.
 
• Referrals to nutritionist, massage therapy, respite care and wellness programs.
 
• Provide access to legal services for matters such as wills and living wills, power of attorney, tax preparation, guardianship and other legal matters related to health and well-being.
HIV/AIDs and other STDs Education
The mission of this program is to increase awareness about HIV/Aids and other STD’s in an effort to change the behavior that spreads HIV and other STD’s, discourage sex at an early age; and promote monogamy and abstinence.  

The program targets three high risk groups: adolescents and teens, young adults, and parents with children that are at high risk of contracting HIV and/or other STD’s. The aim is to create an opportunity for dialogue and education to arm them with the information needed to make the right decisions.

Youth Empowerment/ Virtues for Young Men

Our goal is to help young boys to learn the social etiquette, personal grooming, and table manners that can help them to become confident, self-assured young gentlemen. Young boys are shown the importance of first impressions, personal style, and good posture when they sit, stand and walk.
They will also learn about the importance of a handshake and tips on making introductions and conversation. The program also teaches telephone manners, table manners and other simple courtesies, including earning to say the magic words of "please", "you're welcome" and "thank you".
